I need to get a passport for my 15 yr old daughter. Her father's name is not on the birth certificate, will the passport agency even issue her a passport without his name being on there. (my name is the only name listed as Mother - Father is blank) And if I get a Passport for a minor, does it list both parents? I am just wondering if his name is not on the passport, then I won't need a letter? I took her to mexico when she was about 3 yrs old, but I can't remember how it was handled. Got a passport for my son last year, no father's name listed. Not a problem. Since you send the birth certificate in with the application, they can see for themselves. Good luck!
