It could make a difference if you went, yourself, Steve. After all, it's your application, and who better to say you're one and the same, than yourself. Just changing the "face" can make a difference, when things get stuck, like this has, and going without your wife would change the human dynamic, perhaps just enough to get this done.

Change of Address I went online to do a change of address notification and couldn't find a "tramite" for it on the website. Has anybody had recent experience with this process? Is a change of address notification to INM even required, under the rules in place since May 1, 2010?
Certainly used to be when we last changed address several years ago now though. I'd be surprised if it has changed. It's easy to find out for sure though, just pop down the immigration office queue up for an hour in each of 3 different queues, repeat once a week for 3 weeks and then you'll probably have 3 different answers. Simply pick the one you like the best
Returned to immigration today to tell them that I couldnt get a letter from my bank that confirmed I was the same guy as the person named on the bank statements I had provided. Took along bank cards and driving licence with photo that showed matching signatures, similar names although different variations Stephen Wright, Mr S Wright, Stephen Alan Wright, Mr S A Wright and explained that there is no real 'standard for how names are written out in the UK and that by cross referencing account numbers, signatures and photos they ought to be convinced it really was me. We saw a different person (woman) who then called over the person we had dealt with last time. They discussed it for a few minutes and eventually the guy agreed with the girl that it was a legit application after all. It probably helped that we had a whiny 6 yr old and a baby with us. So now I have to check the Internet when to go back again, presumably to submit proof of payment. Does anyone know if the photos for the plastic card thingy are taken there via the equipment that produces the cards (like they do for driving licenses)? Or do I need to have separate photos taken?
Congrats! You're in, Steve! I'm convinced it was your personal appearance, vouching for your own bona fides, that did the trick. The photo requirement is the same as before- color, size infantil, 3 front view, 2 side view.
Presumably when you present the proof of payment, they will fingerprint you and get a couple of signatures. You will then be told to return very late that day or the next day after 1 pm.
White background, btw. I haven't been doing this, but it was suggested to me when I mboved here. Print your ATM withdrawls and throw them, all the time, into a shoe box. Sort of proves your spending money here and that does count! Photo Guillermo near ADO makes you look like a model, even without make up!
