For anyone wondering, these are the instructions, and it is one form per person: Keep in mind you will need your passport info (passport number, date of issue, date of expiration) the address where you'll be staying (resort address) and what state (if you'll be in Cancun, the state is Quintana Roo)

USA Transfers has an app available. I suggest downloading it. You can message the company directly through it if there are any changes needed or simply to verify your ride is reserved. They have been very responsive any time I have needed them.
Did you try doing it again? You have to submit the printout so maybe just try again and print the correct one?
NO. There are some copy-cat websites that charge a fee for the same document. I think I posted that was a fee one a few posts later. Our last trip we just filled out the card on the plane when handed out by a flight attendant. Airlines are still handing them out. The webpage takes much longer than spending a couple minutes on the plane. Everything has to be entered multiple times. Use this one for no fee. Mexico Tourist Card
