Staple it into your passport before you leave the counter.....and be gentle when you leave after your vacation to hand it back to the people that want it. I work for DMV in Wis.....I want to see the foreigner I 94 form stapled into their passport, witht proper stamping and dates involved.....
Technically stapling your passport is defacing it which you should not do. Paper-clipping it to your passport is a good idea though.
